Position Overview

As a Community Hospice Nursing Specialist at Sharon S. Richardson Hospice, you will be entrusted with delivering compassionate nursing care to patients facing terminal illnesses. This role involves supporting families during challenging home care circumstances, ensuring a holistic approach to end-of-life care. You will be part of a dedicated hospice team, contributing significantly to the quality of life for patients and their loved ones. This is a full-time position with a standard schedule, offering flexibility in hours and minimal on-call responsibilities.

KEY RESPONSIBILITIES

  1. Understand and implement the policies and procedures of Sharon S. Richardson Hospice.
  2. Utilize infection control practices effectively.
  3. Prioritize daily tasks while considering the needs of patients and healthcare professionals.
  4. Maintain organized and safe environments for patient care.
  5. Document patient interactions and care promptly in accordance with established protocols.
  6. Respond effectively to emergency situations.

PATIENT CARE

  1. Oversee the planning, coordination, and delivery of care for patients and their families.
  2. Provide both direct and indirect nursing support to patients and families.
  3. Execute physician orders and maintain open communication with primary care providers.
  4. Educate family members on the signs of approaching death and the grieving process.
  5. Engage with the Interdisciplinary Team (IDT) through regular meetings to ensure comprehensive care.
  6. Act as a liaison to community resources to address patient and family needs.
  7. Conduct thorough assessments using observation, interviews, and data collection.
  8. Monitor and interpret changes in patient behavior and health status for ongoing assessments.
  9. Develop individualized nursing diagnoses and care plans, adjusting as necessary based on patient evaluations.
  10. Provide comprehensive care, including personal hygiene, comfort measures, and emotional support.
  11. Accurately document medications administered, care provided, and patient assessments.
  12. Perform additional duties as required.

HOME CARE MANAGEMENT

  1. Coordinate care among the IDT for a designated group of community patients.
  2. Assess patient outcomes related to self-care and adjust care plans accordingly.
  3. Anticipate and address the needs of patients and families through education and planning.
  4. Schedule home visits with patients and families.
  5. Instruct family members on patient care techniques, medication management, and equipment usage.
  6. Delegate case management responsibilities during absences.

SUPERVISORY ROLE

  1. Guide and support volunteers and ancillary staff based on patient and family needs.
  2. Mentor nursing students and contribute to their evaluations.

PROFESSIONAL GROWTH

  1. Obtain CHPN certification within three years of employment.
  2. Participate in ongoing education and team meetings provided by Sharon S. Richardson Hospice.

  2. Mental Requirements: Proficient in English; capable of interpreting patient conditions and applying nursing procedures; basic math skills for medication calculations; ability to work independently.
  3. Environmental Conditions: Fast-paced, dynamic work environment with multiple directives; home visits with varying conditions.
  4. Equipment Used: IV equipment, glucometers, computerized systems, and other medical devices.
  5. Exposure Risks: Potential exposure to infectious materials and hazardous environments; use of personal protective equipment is required.

QUALIFICATIONS

  1. Licensure: RN license in Wisconsin; valid driver's license; CPR certification.
  2. Experience: Minimum one year in medical/surgical, ICU, public health, home health, or hospice nursing preferred.
  3. Education: Preferred bachelor's degree in nursing or equivalent experience.
  4. Communication Skills: Ability to convey information clearly to patients, families, and team members; recognize and respond to non-verbal cues.
