You will be part of a dedicated hospice team, contributing significantly to the quality of life for patients and their loved ones. This is a full-time position with a standard schedule, offering flexibility in hours and minimal on-call responsibilities.
KEY RESPONSIBILITIES
- Understand and implement the policies and procedures of Sharon S. Richardson Hospice.
- Utilize infection control practices effectively.
- Prioritize daily tasks while considering the needs of patients and healthcare professionals.
- Maintain organized and safe environments for patient care.
- Document patient interactions and care promptly in accordance with established protocols.
- Respond effectively to emergency situations.
PATIENT CARE
- Oversee the planning, coordination, and delivery of care for patients and their families.
- Provide both direct and indirect nursing support to patients and families.
- Execute physician orders and maintain open communication with primary care providers.
- Educate family members on the signs of approaching death and the grieving process.
- Engage with the Interdisciplinary Team (IDT) through regular meetings to ensure comprehensive care.
- Act as a liaison to community resources to address patient and family needs.
- Conduct thorough assessments using observation, interviews, and data collection.
- Monitor and interpret changes in patient behavior and health status for ongoing assessments.
- Develop individualized nursing diagnoses and care plans, adjusting as necessary based on patient evaluations.
- Provide comprehensive care, including personal hygiene, comfort measures, and emotional support.
- Accurately document medications administered, care provided, and patient assessments.
- Perform additional duties as required.
HOME CARE MANAGEMENT
- Coordinate care among the IDT for a designated group of community patients.
- Assess patient outcomes related to self-care and adjust care plans accordingly.
- Anticipate and address the needs of patients and families through education and planning.
- Schedule home visits with patients and families.
- Instruct family members on patient care techniques, medication management, and equipment usage.
- Delegate case management responsibilities during absences.
SUPERVISORY ROLE
- Guide and support volunteers and ancillary staff based on patient and family needs.
- Mentor nursing students and contribute to their evaluations.
PROFESSIONAL GROWTH
- Obtain CHPN certification within three years of employment.
- Participate in ongoing education and team meetings provided by Sharon S. Richardson Hospice.

- Mental Requirements: Proficient in English; capable of interpreting patient conditions and applying nursing procedures; basic math skills for medication calculations; ability to work independently.
- Environmental Conditions: Fast-paced, dynamic work environment with multiple directives; home visits with varying conditions.
- Equipment Used: IV equipment, glucometers, computerized systems, and other medical devices.
- Exposure Risks: Potential exposure to infectious materials and hazardous environments; use of personal protective equipment is required.
QUALIFICATIONS
- Licensure: RN license in Wisconsin; valid driver's license; CPR certification.
- Experience: Minimum one year in medical/surgical, ICU, public health, home health, or hospice nursing preferred.
- Education: Preferred bachelor's degree in nursing or equivalent experience.
- Communication Skills: Ability to convey information clearly to patients, families, and team members; recognize and respond to non-verbal cues.
