Role Summary
The Admission RN is responsible for evaluating patients' eligibility for hospice services and facilitating their admission when appropriate. This role involves initiating the initial care plan with the approval of the attending physician and hospice medical director, ensuring compliance with organizational policies throughout the admission process.
In addition to admissions, the RN will support Clinical Managers by triaging inquiries from patients, families, or healthcare partners, providing guidance as per established protocols, and ensuring timely, high-quality patient care.
This position collaborates closely with the Intake Coordinator to gather referral information and complete the intake process in alignment with organizational standards.
Key Responsibilities
Clinical Duties:
- Conducts and documents initial assessments to determine terminal status.
- Evaluates referrals for hospice eligibility, ensuring that the organization can meet the needs of patients and families.
- Maintains knowledge of regulations and Medicare guidelines.
- Educates patients and families on Advance Directives and confirms existing DNR status.
- Utilizes hospice election forms and consent documentation effectively.
- Initiates necessary services, including physician orders and medical procedures, to address immediate patient needs.
- Facilitates the admission process in collaboration with the interdisciplinary team (IDT).
- Communicates effectively with team members, providing written reports and utilizing a complex care process for multifaceted cases.
- Participates in holiday rotation with RN case managers.
- Performs additional duties as assigned.
Business Development Activities:
- Advocates for the mission of the organization through collaboration with healthcare facilities and community organizations.
- Builds relationships within the healthcare community to enhance referral opportunities and promote the benefits of hospice services.
- Maintains direct communication with facilities regarding referral activities and addresses any quality concerns.
- Contributes to the development and execution of annual plans to achieve organizational goals.
- Exemplifies a positive attitude and upholds the organization's customer service principles.
- Adheres to all organizational policies and procedures.
- Performs other duties as required.
Professional Qualifications:
- Licensure: Must possess a valid RN license to practice in Wisconsin; valid driver's license and car insurance required.
- Education: Minimum of three years of nursing experience in direct patient care, preferably in Palliative or Hospice Care. A four-year degree is preferred, but a two-year degree with relevant experience will be considered.
- Experience: Three to five years of professional experience in development, customer relations, or nursing, demonstrating increasing responsibility.
- Communication Skills: Strong written and verbal communication abilities, capable of providing clear instructions to team members and effectively communicating patient conditions to families and physicians.
- Technical Skills: Proficient in Microsoft Office and experienced with electronic charting systems.

- Mental: Must be able to read, write, and comprehend English; possess cognitive skills to assess patient conditions and apply nursing procedures.
- Environmental: Fast-paced environment with varying demands from multiple disciplines, requiring adaptability.
- Equipment: Familiarity with medical equipment and technology is essential.
- Exposure: Potential exposure to infectious materials and hazardous conditions.
