POSITION SUMMARY:

Illinois Action for Children is in search of a dedicated Family Engagement Case Coordinator to deliver case management services for families with children (preK to third grade) who are frequently absent from school. Reporting to the North Lawndale Community Connections Manager, the Family Engagement Case Coordinator will collaborate with families and educational institutions to identify and mitigate barriers to regular attendance. This role also involves developing strategies in partnership with families, children, and community stakeholders to raise awareness about the significance of consistent school attendance. Furthermore, the coordinator will build relationships with additional North Lawndale families, local schools, and partner organizations to bolster community system development and enhance outcomes.

The ideal candidate will be a compassionate, service-oriented professional committed to achieving positive outcomes for vulnerable children and families in North Lawndale. The candidate should be adept at creating effective case management systems and maintaining communication with families, children, and partners. A positive outlook and a genuine appreciation for the North Lawndale community and its families are essential.

This role requires adherence to our core workplace capabilities, including accountability, customer focus, continuous quality improvement (CQI), effective communication, competent personnel, and clear priorities. All tasks must be executed with minimal supervision and in accordance with organizational standards.

KEY RESPONSIBILITIES:

  1. Manage a caseload of North Lawndale families with children facing chronic absenteeism:
    1. Conduct in-person meetings with families at least twice monthly
    2. Emphasize the importance of regular attendance and early education
    3. Engage with families to identify and resolve challenges affecting attendance
    4. Connect families and children to community resources and document referrals
    5. Assist families in establishing and achieving personal goals for enhanced self-sufficiency
    6. Follow up with families and agencies to document services until case closure
    7. Maintain accurate records of all relevant data points related to case management
    8. Ensure confidentiality of all information managed by IAFC.
  2. Collaborate with team members and partners to develop systems addressing attendance barriers:
    1. Identify high-need families as early as possible
    2. Create and maintain a directory of community resources
    3. Monitor challenges to implementation for team reflection and improvement
    4. Work with the team to design responsive strategies for emerging challenges
    5. Partner with schools to establish a Parent Attendance Committee
  3. Support community system development and collaboration within the Innovation Zone:
    1. Foster ongoing relationships with North Lawndale families, children, and partners
    2. Promote and connect children aged 0-5 to formal early learning programs
    3. Organize field trips for North Lawndale families
    4. Meet with community resource partners bi-monthly to facilitate referrals and serve as a connector and information resource
    5. Represent IAFC at external events and organizations to promote agency services
    6. Regularly attend program-related events

QUALIFICATIONS:

To excel in this role, candidates must demonstrate the ability to fulfill each essential duty satisfactorily. The qualifications listed below are representative of the knowledge, skills, and abilities required:

  • Bachelor's Degree in Social Work, Early Childhood Education, Family Studies, or a related field, with two years of experience in a caseworker or similar role. Previous experience working with children and families is essential.
  • Experience engaging with culturally diverse families and communities, demonstrating cultural sensitivity and appropriateness.
  • Exceptional written and verbal communication skills, with the ability to establish rapport at all levels.
  • Strong documentation skills.
  • A deep respect for confidentiality, involving families and colleagues.
  • Experience in motivating others to achieve their goals.
  • Willingness to work flexible hours, including evenings and some weekends.
  • Proficiency in Microsoft Word, Excel, and PowerPoint; comfort with data entry.
  • A valid Driver's License is required, along with reliable personal transportation and insurance.
