We are seeking a highly skilled and compassionate Community Engagement and Resource Coordinator to join our team at Marillac St. Vincent Family Services Inc. As a key member of our Community Outreach Services department, you will play a vital role in providing full-cycle support services to vulnerable populations, including intake, establishing care plans, and delivering successful service interventions to achieve individualized goals.
Key Responsibilities- Person-Centered Support Services: Coordinate and deliver person-centered support services, including participant engagement, goal development, and delivery of service interventions to support achievement of participant goals and expected program outcomes.
- Participant File Management: Maintain accurate and timely participant files and documents, including documentation of direct and indirect service delivery, case notes, and other requested and/or supporting documentation.
- Community Partnerships: Create and maintain a network of community partners that enhance supports available to participants and increase access to additional community-based services.
- Community Resource Events: Oversee monthly community resource events related to one or more of the Social Determinants of Health.
- Collaboration and Communication: Maintain collaborative working relationships with MSV staff in other program departments to make wrap-around services accessible for adults, families, seniors, and other identified individuals.
- Community Food Source Services: Support with Community Food Source services, including engaging clients one day a week during food distribution operations.
- Workshops and Education: Initiate and facilitate workshops related to community needs and engage in ongoing education and professional development.
- Program Evaluation and Improvement: Evaluate and improve support services' efficiency and effectiveness through developing, implementing, and analyzing benchmarks of program success and outcome measures.
To be successful in this role, you will need:
- A minimum bachelor's degree with a focus in human/social services, preferred.
- A minimum of 2 years related work experience with vulnerable populations.
- Effective and excellent interpersonal skills and ability to relate to a diverse population.
- Strong organizational and time management skills.
- Ability to work effectively both independently and as part of a team.
- Excellent communication and computer skills, including proficiency in Microsoft Office applications and data systems.
- Knowledgeable in navigating Benefits Application, Job Opportunities, and Housing Resources.
- Ability to read and speak English proficiently.
