Job Title: Licensed Practical Nurse

**Job Summary:**

We are seeking a skilled Licensed Practical Nurse to join our team at Episcopal Church Home. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met.

Responsibilities:

  • **Develop and Implement Care Plans:** Collaborate with our interdisciplinary team to create and implement individualized care plans that address the unique needs of each resident.
  • **Provide Direct Care:** Deliver high-quality nursing care to residents, including administering medications, performing wound care, and providing emotional support.
  • **Monitor and Document Resident Status:** Continuously assess and document the health status of residents, identifying any changes or concerns that may impact their care.
  • **Communicate Effectively:** Maintain open and effective communication with residents, families, and healthcare team members to ensure seamless care coordination.
  • **Participate in Quality Improvement:** Contribute to our quality improvement initiatives, identifying areas for improvement and implementing changes to enhance resident care and satisfaction.
  • **Supervise and Train:** Supervise and train non-professional nursing staff, ensuring they have the necessary skills and knowledge to provide high-quality care.

Requirements:

  • **Current NYS Practical Nurse Licensure:** Possess a current and active license to practice as a practical nurse in the state of New York.
  • **Clinical Experience:** Have a minimum of one year of clinical experience as a licensed practical nurse.
  • **Effective Communication Skills:** Possess excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
  • **Flexibility and Adaptability:** Demonstrate flexibility and adaptability in a fast-paced healthcare environment, with the ability to prioritize tasks and manage multiple responsibilities.
  • **CPR Certification:** Hold current certification in cardiopulmonary resuscitation (CPR).

Working Conditions:

The majority of duties will be performed indoors and in resident areas. Employees may be exposed to indoor temperature fluctuations.

Supervisory Relationship:

This position requires periodic supervision with some initiative and independent judgment.

Safety Responsibilities:

Must be familiar with all safety policies and procedures, actively participate in fire drills and emergencies, and maintain confidentiality of all organizational information.
