As a Patient Transition Coordinator at AccentCare, you will play a vital role in ensuring optimal outcomes for patients. Your commitment to delivering exceptional care will be at the forefront of your responsibilities.
Role Summary
In this position, you will engage directly with clients, providing tailored care that meets their unique needs. Collaborating closely with healthcare professionals, you will formulate care plans and apply nursing principles to deliver high-quality support to your clients.
Key Responsibilities
1. Facilitate effective communication among physicians, social workers, discharge planners, and agency staff to ensure all parties are informed about patient needs and referral processes.
2. Conduct clinical assessments and relay critical information regarding patient care prior to home care admissions, ensuring a seamless transition.
3. Partner with discharge planners and physicians to perform bedside evaluations, enhancing the transition experience for patients and their families.
4. Maintain strong relationships with patients by providing ongoing support and ensuring a positive experience throughout their care journey.
5. Manage the procurement of physician signatures on care orders and ensure timely processing of these orders within the agency.
Qualifications
The ideal candidate will possess a Bachelor's degree along with three years of relevant experience, or an equivalent combination of education and experience. An advanced degree is preferred, alongside a comprehensive understanding of facility discharge processes. Candidates must hold a valid RN, LVN, or PT license in the practicing state and maintain a current driver's license and liability insurance.
