The Registered Nurse Care Coordinator plays a vital role in overseeing the daily management of patient care, ensuring continuity from the first point of contact through ongoing visits. This position guarantees that patient care services are delivered promptly and at the appropriate level of care. The primary objective of this role is to foster and engage in initiatives that lead to effective patient care and optimal resource utilization throughout the care delivery process.

Minimum Educational Qualifications

  • Graduate from an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.

Minimum Professional Experience

  • A minimum of 2 years of relevant clinical RN experience in the specified specialty area.
  • Experience in a nursing leadership or charge role is highly preferred.
  • Experience in educating diverse populations on healthcare topics is strongly desirable.

Licenses and Certifications Required

  • Current RN license in Vermont.
  • BLS Certification from the American Heart Association.
  • Valid Driver's License (if home visits are part of the role).
  • Department-specific certifications may be required.
