Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at VNA & Hospice of the Southwest Region Inc. as a Patient Care Specialist. In this role, you will provide high-quality, patient-centered care to individuals in need of hospice services.

Key Responsibilities
  • Deliver Quality Care: Utilize the nursing process to provide individualized goal-directed nursing care to patients, ensuring their physical, emotional, and spiritual needs are met.
  • Develop and Implement Care Plans: Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that address the unique needs of each patient.
  • Communicate Effectively: Maintain accurate and timely documentation of patient care, communicate effectively with patients, families, and healthcare teams, and provide education to patients and caregivers on disease management, treatment options, and self-care techniques.
  • Coordinate Care Services: Coordinate with healthcare teams, vendors, and community resources to ensure seamless delivery of care services, including medication management, wound care, and palliative care.
  • Participate in Quality Improvement: Contribute to quality improvement initiatives, participate in peer review audits, and provide constructive feedback to enhance the quality of care provided.
Requirements
  • Education and Licensure: Current LPN licensure and prior clinical acute care nursing experience required. Prior hospice experience preferred.
  • Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records and other healthcare software.
