Alternate Solutions Health Network LLC is seeking a skilled and compassionate Home Health Physical Therapist Assistant to join our team in Fairfax, Virginia.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist Assistant to provide high-quality patient care in a home health setting. The successful candidate will work closely with our interdisciplinary team to develop and implement individualized care plans that promote optimal patient outcomes.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments of patients' physical abilities and develop individualized care plans to address their needs.
  • Implement and modify treatment plans as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with agency policies and procedures.
Requirements:
  • Current licensure as a Physical Therapist Assistant in the state of Virginia.
  • Minimum one year of experience as a Physical Therapist Assistant in a home health setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of schedules, including evenings and weekends.
