Job Summary

We are seeking a highly skilled and compassionate Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes, promoting optimal recovery and independence.

Key Responsibilities
  • Develop and implement individualized plans of care to address patients' physical therapy needs, in collaboration with attending physicians and other healthcare professionals.
  • Conduct thorough assessments, evaluations, and treatments to relieve pain, restore function, and maintain maximum performance, using evidence-based practices and techniques.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
  • Supervise and educate physical therapist assistants and other support staff to ensure high-quality care delivery.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements, maintaining current licensure and certification as a Physical Therapist.
Requirements
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum one year of experience as a Physical Therapist in an acute or home health setting.
  • Valid driver's license and reliable transportation.
  • Ability to lift, bend, and move patients as needed, with physical stamina to perform duties in a fast-paced home health environment.
