Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their goals and maintain their independence.

Responsibilities:
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help clients improve their mobility and function.
  • Continuously evaluate and revise care plans to ensure optimal outcomes and client satisfaction.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document all interactions, interventions, and outcomes in a timely and accurate manner.
Requirements:
  • Current VA Physical Therapist license.
  • Outpatient experience preferred but not required.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
