About the Role

We are seeking a highly skilled and compassionate Home Health Physical Therapy Assistant to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and achieving positive outcomes for our patients.

Key Responsibilities
  • Plan of Care: Develop and implement individualized plans of care for patients, working closely with the attending physician and Physical Therapist to ensure comprehensive and effective treatment.
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Minimize Patient Risk: Assess functional problems and provide intervention and, in conjunction with the Physical Therapist, work toward goals to improve patient functional status.
  • Family Educator/Advocate: Counsel patients and families on the disease/injury and how to manage in all phases of physical therapy. Instruct patients and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Operations: Attend in-service training and mandatory agency meetings.
Requirements
  • Physical Therapist Assistant (PTA) License: Current license in the state of employment.
  • Experience: Minimum one-year experience as a PTA in an acute care setting. Home care experience preferred.
  • Valid Driver's License: Valid driver's license and auto insurance in your name as a driver.
  • Capable of All Physical Demands: Capable of all physical demands.
