Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will work as part of an interdisciplinary team to develop and implement individualized plans of care, ensuring our patients receive the best possible outcomes.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care, taking into account the patient's unique needs and goals.
  • Conduct thorough assessments and evaluations to identify areas for improvement and develop strategies for achieving optimal outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes accurately and in a timely manner.
Requirements:
  • Current licensure as a Physical Therapist in the state of Virginia.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Valid driver's license and auto insurance.
  • Ability to work independently and as part of a team.
