About the Job

The Clinical Health Specialist will work collaboratively with our interdisciplinary team to provide exceptional direct diagnostic and therapeutic patient care. This role requires a strong focus on delivering high-quality, patient-centered care, while maintaining a commitment to professionalism, compassion, and excellence.

Responsibilities
  • Clinical Care: The Clinical Health Specialist will be responsible for assessment, triage, and therapeutic management of patient needs throughout the disease process. This will involve working closely with attending physicians and other healthcare professionals to ensure seamless communication and coordination of care.
  • Patient Education & Psychosocial Support: The Clinical Health Specialist will educate patients and families on care and recommended resources, providing psychosocial support in a caring, ethical, and professional manner.
  • Information Management: The Clinical Health Specialist will ensure accurate documentation within hospital and regulatory requirements, protecting information from unauthorized access or disclosure.
  • Quality Management: The Clinical Health Specialist will demonstrate high standards of moral and ethical behavior, ensuring compliance with patient-related healthcare regulatory requirements and standards.
Qualifications

To be successful in this role, you will need to possess:

  • A graduate degree from an accredited Nurse Practitioner or Physician Assistant Program.
  • Preferable clinical experience as a NP/PA within the given specialty/service line.
  • Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License, when indicated for pediatric position profile PALS, when indicated for neonatal care NRP Certification.
