Key ResponsibilitiesClient Engagement
* Evaluate client needs and establish goals that promote maximum independence within their community.
* Provide skills training to clients and collaborate with them and other team members to explore alternatives to hospitalization, higher levels of care, homelessness, and incarceration.
* Advocate for clients to fulfill their needs, desires, and best interests.
* Employ critical thinking to make independent judgments while maintaining accountability for the services provided.
* Understand the impact of mental illness and relate respectfully to clients with diverse abilities and diagnoses.
* Meet clients in their homes or public locations, ensuring safe transportation in personal vehicles.
* Exhibit teamwork skills, including problem-solving, cooperation, and negotiation.
* Obtain a medication management certificate within the first three months of employment to administer medications to clients in various settings.
* Be available for on-call shifts during evenings and weekends, which includes managing the 24-hour on-call client crisis line and providing crisis triage services.
* Engage in ongoing professional development and participate in activities that enhance service delivery.
Documentation Responsibilities* Complete all required documentation within established guidelines.
* Prioritize client needs while fulfilling documentation requirements.
* Understand and adhere to all funder and accreditation standards.
* Track and document monthly outcome objectives.
QualificationsEducation: A Bachelor's Degree is required, preferably in Human Services or a related field.
Experience: At least one year of experience serving the target population, particularly individuals with mental illness in employment and job-seeking contexts.
Licenses/Certifications: A valid driver's license is required for work-related driving. Ability to obtain a medication management certificate within three months of hire, along with CPR and First Aid training.
Knowledge/Skills/Abilities: Proficiency in speaking, reading, and writing fluent English. Ability to comprehend common terminology in the mental health field and enter documentation in an EHR system while using Microsoft Office. Capability to effectively present information and respond to inquiries from clients, medical professionals, management, staff, and the public.
Other: Familiarity with standard equipment necessary for performing essential job functions.
