Job Title: Community Support Specialist

UnityPoint Health is seeking a Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in providing support and services to individuals in the community, enabling them to live, work, and recreate in a safe and healthy environment.

Responsibilities:
  • Assess the needs of clients and develop goals to achieve independence in the community.
  • Work with clients and team members to explore alternatives to hospitalization, higher levels of care, homelessness, and incarceration.
  • Provide skills teaching to promote independence.
  • Advocate for clients to meet their needs, desires, and best interests.
  • Utilize critical thinking to make independent judgments and maintain responsibility and accountability for services provided.
  • Knowledge of the effects of mental illness and ability to relate respectfully to clients with a wide range of abilities and diagnoses.
  • Ability to meet clients in their homes or public places, with the ability to drive safely and transport clients in your own vehicle.
  • Possess teamwork skills, including problem-solving, cooperation, negotiating, etc.
  • Meet established revenue targets.
  • Continue professional development and involvement in professional activities that enhance service provision.
Documentation:
  • Complete all documentation within established guidelines.
  • Ability to prioritize client needs and complete required written documentation.
  • Demonstrate an understanding of all funder and accreditation standards.
  • Track and document monthly outcome objectives.
Qualifications:
  • Education: Bachelor's Degree required, preferred in Human Services or related field.
  • Experience: One year with population served preferred.
  • License(s)/Certification(s): Valid driver's license and insurance when driving any vehicle for work-related reasons.
  • Knowledge/Skills/Abilities: Ability to speak, read, and write fluent English, ability to read and comprehend common terminology of the mental health field, ability to enter documentation in an EHR and use Microsoft Office, ability to effectively present information and respond to questions from clients, medical professionals, management, staff, and the public.
