Family Support Specialist

The Family Peer Support Specialist will play a critical role in coordinating care and providing comprehensive transitional services to clients and their families. This includes following up with monitoring, attending joint staffing treatment meetings, and supporting the coordination of care with primary care providers and specialists.
Key Responsibilities:
- Engage clients and families as an alternative to emergency room or hospital care
- Participate in the development of crisis plans
- Monitor for potential crisis escalation or need for intervention
- Follow up with phone calls and face-to-face visits with clients and families after discharge from the emergency room or hospital
The Family Peer Support Specialist will provide individual and family support services to clients and their families. This includes helping clients overcome barriers to services, providing assistance in accessing needed self-help and peer/family support services, and advocating for clients and families.
Key Responsibilities:
- Help clients overcome barriers to services, including anxiety, financial deficits, transportation issues, health limitations, etc.
- Provide assistance to clients in accessing needed self-help and peer/family support services
- Advocate for clients and families
- Provide family support services for clients and their families
- Assist clients and families in identifying and developing social support networks
- Support Medicaid adherence efforts, as needed
- Identify community resources that will help clients and their families reduce barriers to their highest level of health and success
- Linkage and support for community resources, insurance assistance, waiver services
- Connection to peer advocacy groups, family supports networks, wellness center, NAMI, and family programs
The ideal candidate will have a high school diploma and experience as a parent or primary caregiver of a child with an emotional, behavioral, or mental health need, or individual with experience with a friend or family member who has symptoms of a mental health condition. The candidate will also have experience navigating child-serving mental and behavioral health systems and have completed or will complete a state-sanctioned Family Peer Support Specialist training and passed a competency exam based on the training.
Key Qualifications:
- Experience as a parent or primary caregiver of a child with an emotional, behavioral, or mental health need, or individual with experience with a friend or family member who has symptoms of a mental health condition
- Experience navigating child-serving mental and behavioral health systems
- Completed or will complete a state-sanctioned Family Peer Support Specialist training and passed a competency exam based on the training
