Responsibilities
  • Provide comprehensive support services to families, including care coordination, individual and family support, and advocacy.
  • Develop and maintain effective working relationships with clients, families, and healthcare providers to ensure seamless care coordination.
  • Identify and connect clients and families with community resources, insurance assistance, and waiver services to reduce barriers to their highest level of health and success.
  • Collaborate with the Eyerly Ball team to provide support and consultation to encourage and model positive behaviors with clients and families.
  • Participate in joint staffing treatment meetings and follow up monitoring to ensure continuous improvement in client care, communication, and satisfaction.
Requirements
  • High School Diploma required.
  • Experience as a parent or primary caregiver of a child with an emotional, behavioral or mental health need, or individual with experience with a friend or family member who has symptoms of a mental health condition.
  • Completed or will complete a state-sanctioned Family Peer Support Specialist training and passed competency exam based on training.
  • Possess a valid driver's license and a good driving record.
