Job Title: Bilingual Health Home Care Coordinator

The Korean Women's Association is seeking a highly skilled and compassionate Bilingual Health Home Care Coordinator to join our team. As a key member of our health services team, you will work closely with clients to develop and implement personalized health action plans, ensuring they receive the right care at the right time.

Job Summary

The Bilingual Health Home Care Coordinator will be responsible for assessing client needs, coordinating care services, and facilitating interdisciplinary collaboration to promote improved health outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team-based environment.

Key Responsibilities
  • Assess client needs and develop personalized health action plans
  • Coordinate care services, including referrals and advocacy
  • Facilitate interdisciplinary collaboration and care continuity
  • Engage clients in care coordination activities to promote improved health outcomes
  • Provide evidence-based health assessments and screenings
  • Document client files and maintain accurate records
Requirements
  • Bachelor's degree in a related field (e.g., social work, nursing, health education)
  • 1+ year of experience in a related field (e.g., case management, health services coordination)
  • Ability to speak Cambodian and communicate effectively with clients
  • Valid WA State driver's license and current automobile insurance
  • Ability to pass a background check
Working Conditions
  • Ability to work in a fast-paced environment with multiple priorities
  • Ability to travel to client homes and community agencies
  • Ability to lift and/or move up to 25 lbs occasionally
