Key Responsibilities- Provide direct care and support to clients, including assistance with daily living tasks, medication administration, and behavioral interventions.
- Develop and implement individualized treatment plans, in collaboration with the interdisciplinary team, to address clients' unique needs and goals.
- Monitor clients' physical and behavioral health, reporting any changes or concerns to the supervisor or program manager.
- Assist with household chores, meal preparation, and laundry, while maintaining a clean and safe living environment.
- Supervise family visitation and participate in community-based outings and recreational activities with clients.
- Participate in mandatory training and in-service programs to enhance skills and knowledge.
To be successful in this role, you will need:
- A valid CT driver's license and a good driving record.
- An Associate's Degree or higher in a related field, with at least one year of experience in the human services field.
- Training in Physical Management, First Aid, CPR, and Medication Administration.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and community partners.
