Role Overview:

The Outreach Case Manager plays a crucial role in enhancing our program's outreach to the most at-risk women experiencing homelessness. This position is instrumental in guiding community members from faith-based and business sectors to engage meaningfully with their unhoused neighbors.


Key Responsibilities:

  • Participate in drop-in day space operations once a week.
  • Facilitate the integration of Housing Transition Program services with other Catholic Charities initiatives.
  • Ensure the safety of clients, staff, and property; assist in crisis situations as needed; adhere to established safety protocols and procedures; maintain knowledge of regulations and evacuation protocols.
  • Manage a limited caseload, providing direct case management, advocacy, and support services aimed at securing permanent housing for clients encountered during outreach or utilizing drop-in services.
  • Assist clients in accessing essential community resources, including employment, healthcare, benefits assistance, and addiction treatment services.

Qualifications:

Education and Experience:

  • Bachelor's degree in human services or equivalent experience is required.

Preferred Experience:

  • At least one year of direct experience working with individuals facing homelessness is preferred.
  • Two years of experience in case management, information referral, and advocacy.
  • Willingness to obtain certifications within the first six months of employment, including:
    • Current CPR/First Aid certifications.
    • Narcan training.
    • Blood Borne Pathogens training.
    • Mental Health First Aid certification.
  • Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, and PowerPoint.
