About the Role

We are seeking an experienced Registered Nurse to join our team as a Hospice RN / RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care, education, and support to patients and their families. Your primary responsibility will be to assess patient needs, develop and implement individualized care plans, and collaborate with other healthcare professionals to ensure seamless delivery of care.


Key Responsibilities
  • Assess patient needs and develop comprehensive care plans
  • Provide direct patient care services, including wound care, medication management, and symptom control
  • Collaborate with interdisciplinary teams to ensure effective care coordination
  • Communicate effectively with patients, families, and healthcare providers

Requirements

To be successful in this role, you will need:

  • A valid RN license in the state of employment
  • 1+ years of experience in hospice, home health, or related field
  • Strong assessment, communication, and interpersonal skills
  • Ability to work independently and collaboratively as part of a team

What We Offer

In return for your expertise and dedication, we offer a competitive salary range of $ 90,000 - $120,000 per year, depending on experience.
