Hospice Case Manager RN Job Responsibilities

We seek a dedicated PRN Hospice Registered Nurse to join our team as a Hospice Case Manager RN. Your primary duties will include:

  • Conducting thorough assessments of patients' physical, emotional, and spiritual needs.
  • Developing and implementing individualized plans of care to address these needs.
  • Collaborating with multidisciplinary teams to coordinate care and services.
  • Providing education and support to patients and their families on symptom management and end-of-life care.
Hospice Case Manager RN Requirements

To be considered for this role, you should possess:

  • An active RN license in the state of employment.
  • A minimum of 1 year of experience in hospice, home health, or a related field.
  • Strong communication and interpersonal skills to interact effectively with patients, families, and healthcare professionals.
  • The ability to maintain confidentiality and handle sensitive information with discretion.

The estimated salary for this position is $65,000 - $85,000 per annum, based on experience and location.
