Job Summary

We are seeking an experienced RN to join our team as a Hospice RN. In this role, you will be responsible for assessing patient needs, developing individualized care plans, and coordinating services with other healthcare professionals. You will work closely with our interdisciplinary team to deliver high-quality care that empowers patients and their loved ones to manage the challenges of serious illness with dignity and comfort.

Responsibilities:
  • Assess patient needs and develop individualized care plans.
  • Coordinate services with other healthcare professionals, including physicians, therapists, and social workers.
  • Provide direct patient care services, including wound care, medication management, and symptom control.
  • Educate patients and their families on disease management, pain control, and end-of-life care.

We offer a competitive salary range of $80,000 - $105,000 per year, depending on experience and location.
