As part of our HouseCalls initiative, we deliver annual clinical evaluations to patients in the comfort of their residences. This vital preventive care plays a crucial role in identifying and mitigating health risks, while also coordinating necessary follow-up care to enhance overall health and wellness.

In this position, you will enjoy the flexibility to maintain a work-life balance. We offer adaptable scheduling, allowing providers to manage their time throughout the 7-day work week. There are no on-call duties, weekends, or holiday obligations.

Key Responsibilities:

  • Conduct in-home evaluations, including assessment of medical history, systems review, medication reconciliation, vital signs, and comprehensive physical examinations.
  • Perform evidence-based screenings, including point-of-care testing as necessary.
  • Identify diagnoses for care management and active medical oversight to support treatment.
  • Compile a list of current and historical medical conditions using clinical expertise and assessment findings.
  • Communicate assessment results to inform the member's primary care provider of potential care gaps.
  • Recognize urgent and emergent situations and respond appropriately.
  • Educate members on various topics, including disease processes, medications, and adherence to treatment plans.
  • Travel may be required up to 85% of the time.

Required Qualifications:

  • Active Nurse Practitioner or Physician Assistant license or the ability to obtain it by the start date. Licensure must be unencumbered, with no open or unresolved disciplinary actions.
  • Active national NP or PA certification or the ability to obtain national certification in the state of assignment by the start date (For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA).
  • Capability to meet the physical demands of the role, including lifting a 30-pound bag, navigating stairs, and performing physical assessments in various environments.
  • Reliable transportation for conducting home visit assessments.

Preferred Qualifications:

  • Potential to obtain additional licensure in other geographic regions.
  • 1+ years of clinical experience in the highest level of education or clinical setting.
  • Experience in fields such as gerontology, cardiology, internal medicine, or endocrinology.
  • Experience in home health care or conducting home visits.
  • Ability to travel up to 5% within licensed states if requested for additional incentives.

Important Note: Employees must maintain an active regular status and remain in the role for a minimum of 12 months from the date of hire/rehire/transfer. If an employee departs from Home and Community, student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
