In this position, you will enjoy a balanced work-life dynamic. Flexible scheduling is available, allowing providers to adjust their hours throughout the week. There are no on-call duties, weekend shifts, or holiday work required.

Key Responsibilities:

  • Conduct comprehensive in-home evaluations, including medical history assessment, system reviews, medication reconciliation, vital signs monitoring, and thorough physical examinations.
  • Implement evidence-based screenings, including point-of-care testing as appropriate.
  • Identify diagnoses for care management and active medical oversight to support treatment plans.
  • Compile a list of current and historical medical conditions utilizing clinical expertise and assessment findings.
  • Communicate assessment results to inform the patient's primary care provider of any potential care gaps.
  • Recognize urgent and emergent situations and respond accordingly.
  • Educate patients on various topics, including disease processes, medication adherence, and compliance.
  • Travel up to 85% as required.

Required Qualifications:

  • Possession of an active NP or PA license or the ability to obtain one by the start date. Licensure must be unencumbered and free of any unresolved disciplinary actions.
  • Active national NP or PA certification or the ability to secure national certification in the assigned state by the start date (For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA).
  • Capability to meet the physical demands of the role, including lifting a 30-pound bag, navigating stairs, and performing physical assessments.
  • Reliable transportation for conducting home visit assessments.

Preferred Qualifications:

  • Willingness to obtain additional licensure in other regions if requested.
  • At least 1 year of clinical experience in a relevant healthcare setting.
  • Experience in specialties such as gerontology, cardiology, internal medicine, or endocrinology.
  • Prior experience in home health care or conducting home visits.
  • Ability to travel up to 5% within licensed states if requested for additional incentives.
