In this position, you will enjoy a balanced work-life dynamic. Flexible scheduling is available, allowing providers to adjust their hours throughout the week. There are no on-call duties, weekend shifts, or holiday work required.
Key Responsibilities:
- Conduct in-home evaluations, including a review of medical history, systems review, medication reconciliation, vital signs, and comprehensive physical examinations.
- Perform evidence-based screenings, including point-of-care testing as necessary.
- Identify diagnoses for care management and active medical oversight in support of treatment.
- Compile a list of current and past medical conditions utilizing clinical expertise and assessment findings.
- Communicate assessment results to inform the member's primary care provider of potential care gaps.
- Recognize urgent and emergent situations and respond appropriately.
- Educate members on topics such as disease processes, medications, and adherence to treatment plans.
- Travel may be required up to 85% of the time.

Required Qualifications:
- Current NP or PA license or the ability to obtain one by the start date. Licensure must be unencumbered and free of any unresolved disciplinary actions.
- Active national NP or PA certification or the ability to obtain certification in the state of assignment by the start date (For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA).
- Capability to meet the physical demands of the job, including lifting a 30-pound bag, navigating stairs, and performing physical assessments.
- Reliable transportation for home visit assessments.
Preferred Qualifications:
- Potential requirement for additional licensure in other geographic areas.
- 1+ years of clinical experience in a relevant healthcare setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Background in home health care or home visit services.
- Willingness to travel up to 5% within licensed states if requested for additional incentives.
Note: Employees must maintain an active regular status and remain in the role for a minimum of 12 months. If an employee leaves Home and Community, student loan repayments will cease. Employees must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of student loan repayments.
