Job Description:

We are seeking an experienced RN-Case Manager to join our team at Baptist Memorial Health Care in Oxford, MS.

The successful candidate will be responsible for facilitating the safe transition of patients throughout the continuum of care, ensuring efficient utilization of resources and compliance with federal regulations.

Key Responsibilities:

  • Manage patient care transitions
  • Develop discharge plans
  • Participate in readmission reduction initiatives
  • Communicate with payers to reduce denials

Requirements:

  • BSN or MSN degree in Nursing
  • At least 1 year of clinical experience as an RN
  • Critical thinking skills, communication, and problem-solving abilities

Estimated Salary:$65,000 - $85,000 per annum
