About the Job: We are seeking a skilled Healthcare Transition Specialist to join our team at Baptist Memorial Health Care. As a Healthcare Transition Specialist, you will be responsible for coordinating patient transitions, ensuring timely and effective care, and promoting high-quality patient outcomes.

Key Responsibilities:

  • Coordinate patient transitions: Facilitate smooth transfers between healthcare settings, promoting optimal care and reducing readmissions.
  • Develop discharge plans: Collaborate with healthcare providers to create personalized care plans that meet patient needs and preferences.
  • Promote health education: Educate patients and families on self-care, disease management, and available resources to support ongoing health and wellness.

Requirements:

  • Education: A minimum of a Diploma or Associate Degree in Nursing, with a preferred BSN or MSN.
  • Experience: A minimum of 1 year of clinical experience as an RN, with a preferred 3 years of experience in case management, preferably in a hospital or payer setting.
  • Licensure/Certification: A valid RN license, with a preferred CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification.
