Job Title: Clinical Rehabilitation Specialist

About the Role

Evaluate and assess patients to establish measurable goals that promote restoration of functional abilities. Provide treatment according to a written plan of care to meet established goals.

Plan for discharge and communicate findings and progress according to professional licensure/certification, scope of practice and departmental/discipline policies and procedures.

Produce reports as needed. May supervise clinical students and support staff including certified/licensed assistants, technicians, and volunteers.


Key Responsibilities
  • Evaluate and assess the patient based on subjective and objective information.
  • Communicate results of the evaluation to patient and other caregivers.
  • Provide treatment according to established goals on plan of care.
  • Document interventions/care in the patient's medical record.
  • Evaluate effectiveness of plan of care and reassess patient's functional status as needed.
  • Provide discharge planning information which includes patient/family education, follow-up, and referral to community resources.
  • Contribute to the development of staff and students.
  • Participate in activities designed to improve health care delivery.

Compensation and Benefits

Estimated Salary Range: $62,500 - $87,000 per year.
