Job Title: Case Manager Early Intervention Agency

We are seeking a highly skilled and dedicated Case Manager to join our team at Little Masters Development Center L. As a Case Manager, you will play a vital role in ensuring the success of our Early Intervention program, providing support and services to families and children in the NYS Early Intervention Program.

Responsibilities:
  • Manage a caseload of children receiving Early Intervention services, ensuring compliance with all relevant regulations and guidelines.
  • Develop and implement Individualized Family Service Plans (IFSPs) that meet the unique needs of each child and family.
  • Provide ongoing support and guidance to families, including communication, documentation, and reporting.
  • Collaborate with service coordinators, providers, and other stakeholders to ensure seamless delivery of services.
  • Manage deadlines and priorities, ensuring timely completion of tasks and projects.
  • Maintain accurate and up-to-date records, including digital files and NYEIS documentation.
  • Communicate effectively with families, service coordinators, and other stakeholders, providing clear and concise information.
  • Participate in training and professional development opportunities to enhance skills and knowledge.
Qualifications:
  • High school diploma or equivalent required; bachelor's degree preferred.
  • Experience working with children and families in an Early Intervention setting.
  • Strong organizational, technical, and communication skills.
  • Ability to work independently and as part of a team.
  • Passion for serving families and children in the NYS Early Intervention Program.
Benefits:
  • Competitive salary range: $44,000 - $57,000 annually.
