Position Overview: The Mental Health Case Coordinator is responsible for delivering tailored support services to individuals with serious mental illness and substance use disorders. This role requires a Bachelor's Degree in a relevant human services discipline and a minimum of two years of experience in case management.

Key Responsibilities:

  • Deliver personalized assistance to empower clients in developing essential life skills for independent living.
  • Conduct comprehensive assessments as part of the ACT framework.
  • Collaborate with the ACT team to determine appropriate services based on client evaluations and immediate needs.
  • Engage in clinical discussions during ACT team meetings, providing insights on client progress and challenges.
  • Utilize motivational interviewing techniques to encourage clients to leverage their strengths in addressing personal challenges.
  • Identify and mitigate risk factors, including substance use and social isolation, through targeted case management strategies.
  • Facilitate access to benefits and income support by assisting clients with appointments related to Medicaid, Medicare, and other financial assistance programs.
  • Encourage client participation in community resources and activities, such as education, vocational training, and recreational opportunities.
  • Involve family members and significant others in the treatment process to enhance support systems.
  • Respond to crisis situations as required, adhering to the established 24/7 coverage protocol.
  • Contribute to the development of comprehensive service plans and participate in regular team meetings.
  • Engage in ongoing training and professional development as mandated by the organization.
  • Assist in the completion of various assessments and maintain accurate documentation of client progress.
  • Work collaboratively with clients' support networks and service providers to ensure comprehensive care.
  • Complete monthly face-to-face interactions with clients in the community.
  • Address individual client needs related to wellness, housing, education, and employment.
  • Provide culturally sensitive services to meet the diverse needs of clients.
  • Utilize necessary computer skills for reporting, documentation, and data management.
  • Fulfill additional responsibilities as assigned within the ACT program.
