Responsibilities:
* Collaborate with members, the Co-op council, and other committees to enhance the ability for members to stay living independently in the co-op.
* Conduct psychosocial assessments for residents on a voluntary basis who may be at-risk or frail.
* Help transition applicants for residency by determining their current and potential needs.
* Coordinate and/or provide general case management and referral services.
* Maintain accurate, complete, updated files on members, programs, and activities through an online case management program.
* Participate in regular meetings with the Co-op team and the Civic Works service coordinator team and provide appropriate status reports.
* Establish relationships with local service providers who are currently providing or could provide services to residents who are at-risk or frail.
* Assure that all service providers are "vetted" based on established criteria prior to making referrals.
* Assist members to create and maintain informal support networks consisting of other members, family, and friends in their Co-ops and the community.
Requirements:
* Associate degree or higher in social work, psychology, gerontology, counseling, or related specialty.
* 1+ years of experience working with older adults in a community-based setting or equivalent combination of education and experience.
* Must have knowledge of senior community and area resources.
* Demonstrated ability to work independently to achieve desired outcomes with older adults individually and in groups.
* Good oral, written, and computer skills required.
