Job Title: Clinical Care Specialist

We are seeking an experienced APRN/Nurse Practitioner to join our Adult Medicine clinic in Hartford, CT.

Job Summary:

The successful candidate will provide comprehensive primary health care, including health promotion and disease prevention activities, to patients in a collaborative environment. The incumbent will work closely with department physicians and refer patients to consultants as needed.

Responsibilities:
  • Provide high-quality, comprehensive clinical care to patients according to established standards, protocols, and practices.
  • Assess medical conditions, interpret findings, and make evidence-based clinical decisions.
  • Communicate effectively with patients, peers, and other stakeholders both orally and in writing.
  • Maintain timely, thorough, and accurate written clinical documentation.
  • Collaborate with department physicians and refer patients to consultants as needed.
Requirements:
  • Graduate of an accredited school of nursing and completion of an approved nurse practitioner program.
  • Valid State of Connecticut licensure.
  • Eligibility to obtain DEA and state narcotic registration.
  • Deep knowledge of nursing theory and practice or physician assistant theory and practice.
  • Ability to provide cultural competent care to a diverse patient population.

Preferred Qualifications:
  • 1-2 years' experience working in an ambulatory setting or providing urgent care.
  • APRN or PA License in the state of Connecticut.
  • Spanish language proficiency.
