Job Duties:
  • Conduct routine check-ups to assess patients' health conditions and identify potential issues.
  • Provide advice on healthy habits, diet, and hygiene to promote overall well-being.
  • Conduct examinations, evaluate symptoms, and reach informed diagnoses based on scientific knowledge and patient medical history.
  • Prescribe and interpret lab tests, medications, and provide comprehensive instructions for administration.
  • Collaborate with physicians, physician assistants, and nurses to form a high-performing medical team.
  • Examine and treat injuries, and refer patients to other physicians when necessary.
  • Keep accurate records of patients' diseases, operations, and significant information.
Qualifications:
  • Unrestricted Nurse Practitioner License in Connecticut required.
  • 2+ years of experience as an NP or a combination of relevant education and experience required.
  • Palliative care, hospice, and/or geriatric experience required.
  • Prescriptive Authority required.
  • Active DEA in Connecticut required.
  • Valid Driver's License required.
  • Reliable personal vehicle required.
  • Up-to-date TB and titers required.
  • BLS required.
  • ACLS required.

RANGE: $110,000-$175,000/year (Salary + Bonus/Profit Sharing)
