Position Overview:
This is a full-time role with a Monday to Friday schedule during daytime hours.
Key Responsibilities:
- Act as a liaison with insurance case managers.
- Manage and coordinate discharge planning for sub-acute residents.
- Work collaboratively with the interdisciplinary team to establish and achieve care goals for sub-acute residents.
Qualifications:
- Background in Nursing or Therapy is required.
- Experience in Long-Term Care is essential.
- Strong organizational and communication abilities.
- Attention to detail and effective follow-through skills.
- Capability to prioritize and manage multiple tasks.
The ideal candidate will possess experience in working with insurance case managers, along with a solid understanding of case management and PDPM, ensuring optimal reimbursement outcomes.
