About the Role

We are seeking a highly skilled and organized Patient Care Coordinator to join our team at UnitedHealth Group. As a Patient Care Coordinator, you will play a critical role in ensuring the smooth operation of our healthcare services. Your primary responsibility will be to coordinate patient care activities, including scheduling appointments, managing patient records, and communicating with healthcare providers.

Key Responsibilities
  • Coordinate patient care activities, including scheduling appointments and managing patient records
  • Communicate with healthcare providers, patients, and family members to ensure seamless care coordination
  • Manage and maintain accurate and up-to-date patient records
  • Assist with data entry and reporting as needed
  • Provide exceptional customer service to patients, families, and healthcare providers
Requirements
  • High school diploma or equivalent required
  • 1+ years of experience in a healthcare setting, preferably in a patient care coordination role
  • Excellent communication and organizational skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Basic computer skills and proficiency in electronic medical records (EMRs)
