Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will work closely with patients, families, and healthcare teams to provide high-quality care and support. As a Hospice RN Case Manager, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with interdisciplinary teams to ensure seamless care delivery.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Coordinate with interdisciplinary teams to ensure seamless care delivery
  • Provide direct patient care services, including wound care, medication management, and symptom management
  • Empower patient caregivers through education and support

Requirements
  • Current RN licensure in the state of employment
  • 1+ years of experience in hospice, home health, or related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
