Hospice Nurse Case Manager

Job SummaryWe are seeking a full-time Hospice RN Case Manager to join our team in the Philadelphia, PA area. As an integral part of our interdisciplinary team, you will work closely with the Director of Nursing and other healthcare professionals to provide comprehensive nursing care that prioritizes the well-being of our patients and their families.
Key Responsibilities- Manage a caseload of patients within your assigned territory, ensuring timely and effective care delivery.
- Conduct thorough nursing assessments and develop personalized care plans to meet the unique needs of each patient.
- Document all patient information accurately and maintain up-to-date records.
- Collaborate with physicians and other staff to optimize patient care and outcomes.
- Educate patients and families on best care practices, promoting self-advocacy and empowerment.
- Attend care conferences and provide support as needed, ensuring seamless care coordination.
- Supervise and support other nursing staff, fostering a culture of teamwork and excellence.
- Participate in training programs and community events, staying up-to-date on industry best practices and advancements.
- Valid RN license in Pennsylvania.
- Previous experience in hospice care, with a strong understanding of palliative care principles.
- Excellent assessment and communication skills, with the ability to work effectively with diverse stakeholders.
- Ability to work well in a team environment, with a strong focus on patient-centered care.
- Valid driver's license and reliable vehicle, with a willingness to travel as needed.
