Job Description

We are seeking a skilled Case Coordinator to join our team at JEVS Human Services. As a Case Coordinator, you will provide intensive case management support to clients, helping them achieve their goals and overcome barriers to self-sufficiency.

Key responsibilities include:

  • Providing in-home and on-site case management support to a caseload of DPW recipients
  • Matching and linking clients with services, including initial telephone calls and orientation to various agencies and personnel
  • Documenting all contacts with clients in a timely manner and maintaining accurate records
  • Maintaining the integrity and confidentiality of client information
  • Developing and utilizing Service Plans as the primary case management tool for each participant
  • Providing regular, ongoing individual counseling to address barriers and encourage achievement of goals
  • Scheduling clients to participate in workshops, job search, and community service to achieve a 50% Work Participation Rate

Requirements include:

  • Bachelor's degree preferred or minimum of 3 years of human service/case management experience
  • Ability to problem solve and collaborate with a variety of customers in the Welfare to Work Industry
  • Proficient in MS Office applications, especially MS Word and Excel
  • Willingness to work a flexible schedule and travel to inner city locations
