Job Summary:

The Case Manager will work closely with clients to assess their needs, develop personalized service plans, and connect them with essential resources. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct home visits and assessments to identify client needs and develop service plans
  • Provide case management services, including crisis intervention, advocacy, and resource connection
  • Collaborate with school personnel, social workers, and service providers to ensure seamless support
  • Offer flexible support outside of regular working hours to accommodate client needs
  • Participate in staff meetings, supervision, and training sessions to stay up-to-date on best practices
Requirements:

  • Bachelor's degree required
  • Mandated Reporter Certificate (within 5 years)
