Job Summary:

As a Case Manager at The Kintock Group, you will play a critical role in providing intake, assessment, and case management services to our clients. Your primary responsibility will be to assess the needs of clients and develop comprehensive plans of care that meet their unique requirements. This will involve providing information on community services and agency services, making referrals on behalf of clients, and educating clients on program participation, including rights and responsibilities and appeal rights. You will also be responsible for utilizing case management skills, such as brokering, advocacy, monitoring, and discharge planning to link clients and their families to resources and services. Additionally, you will work closely with clients to develop and revise care plans that are client-focused and meet client needs.

Key Responsibilities:

* Develop comprehensive plans of care that meet the needs of clients
* Provide information on community services and agency services
* Make referrals on behalf of clients
* Educate clients on program participation, including rights and responsibilities and appeal rights
* Utilize case management skills, such as brokering, advocacy, monitoring, and discharge planning
* Work closely with clients to develop and revise care plans
* Schedule and/or participate in Case Conferences with clients/caregivers
* Provide discharge planning to dis-enrolled clients
* Maintain client physical files with required forms
* Ensure all services are supported by care plans and case note documentation
* Ensure all court stipulations are met by each resident assigned to the caseload
* Facilitate required Cognitive Behavioral Workshops and classes
* Perform other work-related duties as assigned by the Senior Case Manager or Director

Requirements:

* Excellent oral and written communication skills
* Minimum six-months of relevant experience
* A valid driver's license
* Bi-lingual (English/Spanish) preferred

Knowledge:

* Microsoft Office Suite (Microsoft Word, Excel, Outlook)
* ACA Requirements

Abilities & Goals:

* Ability to follow course curriculum, facilitate groups and/or workshops
* Learn, analyze, and interpret understand and follow contract rules, regulations, policies, and procedures
* Participate as an effective member of a program team

Education Requirement:

* High School Diploma (or equivalent)
* Bachelor's degree in Criminal Justice or a Social Science is required
