Job Title: Program Manager - Community Services Coordination

Service Coordination, Inc. (SCI) is seeking a highly skilled and experienced Program Manager to lead our Community Services Coordination program. As a key member of our leadership team, you will be responsible for overseeing the program's operations, ensuring regulatory compliance, and driving programmatic initiatives to achieve strategic goals.

Key Responsibilities:
  • Monitor key performance metrics, including quality compliance, retention, and financial health, to ensure high-quality person-centered services.
  • Develop and implement program priorities, including novel approaches to provide high-quality person-centered services.
  • Provide regular supervision, coaching, and development to a team of supervisors to balance workload and effort.
  • Support supervisors in maintaining service records, person-centered plans, and monitoring service delivery and health and safety for each person supported.
  • Deploy resources efficiently and effectively toward organizational goals.
  • Ensure contractual and regulatory requirements are met.
  • Proactively identify opportunities to improve program efficiencies and develop sound recommendations based on evidence-based analysis.
  • Facilitate a culture of learning, empowering supervisors to make informed independent decisions.
  • Review monthly Quality Assurance data to identify themes and trends and coordinate with Quality Systems to support continuous program improvement.
Requirements:
  • Bachelor's degree in a human service-related field required; Master's degree preferred.
  • At least 3 years of experience in a leadership role at a case management entity.
  • General understanding of core business functions, including project management, accounting and finance, marketing and communications, and legal issues.
  • Ability to work in a mobile work environment.

  • Strong belief in the provision of community services for people being served by the CCS Program.
  • Proven track record of building relationships and formulating partnerships with external contacts.
  • Ability to effectively facilitate work group and committee meetings.
  • Ability to work collaboratively with different groups or service systems to identify, coordinate, and assure appropriate services.
