Job Title: Home Health Speech Language Pathologist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Speech Language Pathologist to join our team in Burke, Virginia.

Job Summary:

We are looking for a compassionate and experienced Speech Language Pathologist to provide high-quality home-based skilled speech therapy care for patients. As a Home Health Speech Language Pathologist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and improve functional outcomes.

Responsibilities:
  • Develop and implement individualized plans of care for patients with speech and language disorders.
  • Provide home-based skilled speech therapy services to patients, including assessment, treatment, and education.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
Requirements:
  • Current license as a Speech Language Pathologist in the state of employment.
  • Minimum one year of experience as a Speech Language Pathologist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job, including traveling to patient homes.
