Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Burke, Virginia.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with our company, you will have the opportunity to work with a diverse patient population and make a meaningful difference in their lives.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes accurately and in a timely manner.
Requirements:
  • Current licensure as a Physical Therapist in the state of Virginia.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Valid driver's license and auto insurance.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
