Job Title: Home Health Speech Therapist

We are seeking a skilled Home Health Speech Therapist to join our team at Alternate Solutions Health Network LLC.

About the Role:
  • As a Home Health Speech Therapist, you will provide skilled speech therapy care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized treatment plans.
  • You will educate patients and their families on speech therapy techniques and strategies to improve communication and swallowing skills.
  • Our ideal candidate will have a strong background in speech therapy, excellent communication skills, and a passion for delivering high-quality patient care.
Responsibilities:
  • Develop and implement individualized speech therapy plans for patients, including assessment, goal setting, and treatment.
  • Provide skilled speech therapy services to patients in their homes, including assessment, treatment, and education.
  • Collaborate with our interdisciplinary team to ensure comprehensive care and coordination of services.
  • Document patient progress and outcomes in accordance with agency policies and procedures.
  • Participate in ongoing education and training to stay current with best practices in speech therapy.
Requirements:
  • Master's degree in Speech-Language Pathology or related field.
  • Current licensure in the state of employment.
  • Minimum one year of experience as a speech therapist in a home health or acute care setting.
  • Valid driver's license and auto insurance.
