Job Summary:We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.
Responsibilities:- Conduct thorough assessments of patients' physical and functional abilities to identify areas for improvement.
- Develop and implement individualized plans of care that address patients' specific needs and goals.
- Provide skilled occupational therapy services, including exercises, activities, and education, to promote patient independence and mobility.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
- Current licensure as an Occupational Therapist in the state of Virginia.
- Minimum one year of experience as an Occupational Therapist in a home health or acute care setting.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team.
