Position Summary
Under the guidance of the Day Center Manager, the Activities Coordinator is tasked with overseeing the Day Center's engagement initiatives and ensuring the Activities Team effectively implements the program. This role involves the creation and execution of daily, monthly, and special event activities, along with therapeutic programs aimed at enhancing or preserving physical, cognitive, social, emotional, and spiritual well-being to promote active participation in life.

Key Responsibilities
- Collaborate as a member of the interdisciplinary team (IDT) and participate in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and communicate changes in participants' baseline status to relevant staff daily.
- Provide insights to the Day Center Manager regarding outcomes of care plan interventions.
- Organize group and individual activities that cater to participants' cognitive, physical, social, psychosocial, spiritual, and artistic needs.
- Develop diverse activities, including exercise sessions, arts and crafts, birthday and holiday celebrations, special events, recreational games, cognitive groups, and community outings.
- Coordinate with other operational departments, including transportation, nutrition services, social services, nursing, and rehabilitation staff.
- Complete timely documentation, including daily records of participant activity participation; initial, semi-annual, unscheduled, and annual assessments; and other participant data as directed by the interdisciplinary team.
- Assist in collecting participant consent forms for photo requests.
- Plan and prepare activities tailored for a dementia-specific population and manage the daily flow of quiet room activities. Support the Day Center Manager in maintaining compliance with assigned responsibilities.
- Uphold confidentiality of participant information and report any concerns or issues directly to the Day Center Manager. Provide coverage for other members of the activities department as necessary.
- Attend Activities Department meetings.
- Assist the Day Center Manager in ensuring all supplies and equipment are available for recreational therapies.
- Prepare and submit inventory reports to the Day Center Manager to facilitate appropriate procurement and cost-effective budgeting. Aid in developing Recreational Activities Satisfaction Surveys.
- Assist in preparing reports on activity utilization.
- Support the Day Center Activities Manager in providing training and guidance to Activities Assistants.
- Demonstrate workplace behavior consistent with organizational core values, including honesty/integrity, respect for others, patient-centeredness, encouragement, and high-quality care.
- Participate in staff meetings, in-services, projects, and committees as assigned.
- Adhere to and support the center's practices, procedures, and policies, including assigned break times and attendance.
- Accept assigned duties cooperatively and perform all other related tasks as assigned.
- Maintain flexibility in work hours.
- May require the use of a personal vehicle.

Education & Training
- A minimum of two (2) years of demonstrated successful experience in activities or therapeutic recreation services with the geriatric population.
- At least one (1) year of documented experience working with a frail or elderly population.
Education and Certification
- A High School Diploma or GED is required at a minimum.
- Spanish-speaking candidates are preferred.
- Experience in planning and hosting events is beneficial.
- Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Skills & Abilities
- Strong knowledge of activities and recreational therapy techniques suitable for the elderly to support the Activities department.
- Advanced understanding of the principles and techniques involved in interacting with frail elderly individuals.
- Creative skills to plan, develop, and implement varied activities that meet diverse participant needs.
- Ability and experience in working with volunteers and vendors.
- Proficient in communicating with participants, family members, and staff; capable of establishing a supportive, trusting relationship with participants.
- Ability to work effectively and harmoniously with staff, elderly individuals, the public, governmental agencies, and service providers.
- Well-organized, dependable, flexible, and resourceful.
- Effective oral and written communication skills.
- Ability to encourage participants to make their own choices.
- Capable of presenting programs to groups of participants.
- Foster social interaction among staff and participants.
- Assist in various small group activities.

- Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; assistance from another qualified employee is required when lifting or transferring objects over 25 pounds.
- Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking, and repetitive leg and arm movements, with occasional bending, reaching forward and overhead, squatting, and kneeling.
- Work is generally performed in an indoor, well-lit, well-ventilated, heated, and air-conditioned environment.
