Position Summary

Collaboration with leadership is essential to enhance care and services across the organization, ensuring both operational efficiency and effectiveness.

Key Responsibilities

  • Provides administrative clinical support and expertise to address the needs of patients and staff, acting as a liaison with physician practices, hospital case management, and other referral sources.
  • Analyzes data to prioritize complex situations, facilitating appropriate actions based on the urgency of patient health, safety, and the effectiveness of service delivery.
  • Identifies obstacles to resolving issues and coordinates with relevant resources or leadership for timely and effective solutions.
  • Reviews and aids in the development of individualized clinical care plans, ensuring clear communication among all parties involved to support quality outcomes and patient satisfaction.
  • Assists clinical staff in executing the established care plan, coordinating service delivery, and implementing strategies that promote health and safety.
  • Monitors progress towards achieving outcomes through documentation reviews, interdisciplinary meetings, and case conferences.
  • Ensures compliance with billing practices by staying informed about payer requirements and regulations, completing workflows promptly, and identifying opportunities for reimbursement.
  • Delegates care responsibilities to appropriate healthcare personnel in accordance with legal and policy guidelines.
  • Engages with patients, families, and other stakeholders in the clinical practice process.
  • Utilizes available resources to plan and deliver clinical services that are safe, effective, and financially prudent.
  • Participates in committees, shared governance, or other decision-making activities within the facility.
Physical Requirements
The role primarily involves office work, requiring extended periods of sitting and computer use. Physical activities may include walking, standing, lifting, reaching, stooping, bending, pushing, and pulling. The ability to lift and support up to 35 pounds is necessary for handling patients and medical supplies. Proficiency in English communication is required, along with intact sensory functions and finger dexterity. Critical thinking skills and the ability to respond swiftly to changes in patient conditions are essential.

Qualifications

Candidates must be graduates of an accredited nursing program, with a BSN preferred. Current BLS certification is required. A minimum of one year of experience in a related field is necessary. A valid RN license or temporary license as a Registered Nurse in the state of practice is mandatory. Maintenance of a current state driver's license and auto insurance is also required. Additional qualifications may be necessary as determined by the nursing leadership.
