POSITION SUMMARY

Support the Nurse Management Team in overseeing Health Center nursing operations to guarantee the provision of exceptional care to residents, adhering to all federal and state regulations.

KEY RESPONSIBILITIES OF THE SENIOR NURSING CARE COORDINATOR

  • Assist in leading and managing nursing staff, including RNs, LPNs, and CNAs, to ensure optimal resident care and satisfaction. Evaluate team performance and provide necessary coaching and training.
  • Oversee wound care protocols, including conducting audits of skin assessments, collaborating with wound specialists, and ensuring compliance with treatment orders.
  • Ensure accuracy in admission documentation and assist nursing staff with admissions and discharges, providing guidance on incomplete documentation.
  • Collaborate with the Health Services Interdisciplinary Team (IDT) to assess and enhance resident care operations while ensuring regulatory compliance.
  • Contribute to the Risk Assessment Committee by preparing reports and recommendations for the Director of Nursing regarding quality assurance and performance improvement initiatives.
  • Assist in investigating resident incidents and completing necessary reports as required.

ADDITIONAL RESPONSIBILITIES

  • Participate in the Nurse Manager On-Call rotation to support nursing staff with critical care decisions.
  • Engage in team meetings for both licensed and non-licensed personnel.
  • Facilitate comprehensive orientation for new nursing team members.
  • Work shifts as needed to ensure adequate resident care.
  • Perform other related duties as assigned.

QUALIFICATIONS FOR THE SENIOR NURSING CARE COORDINATOR

Experience:

  • At least 2 years of experience in skilled nursing within long-term care, assisted living, or hospital settings is preferred.
  • Familiarity with federal, state, and local nursing regulations and experience in interdisciplinary team environments is preferred.
  • Previous leadership experience in healthcare is preferred.

Education/Certifications/Licenses:

  • Current LPN license in the State of Oklahoma is required.
  • Current CPR and First Aid certifications are required.

SKILLS AND ABILITIES

  • Strong knowledge of state and federal regulations related to long-term care.
  • Proficient in care plan development and documentation.
  • Effective communication skills for addressing health issues with residents and families.
  • Ability to manage sensitive information with discretion and professionalism.

PHYSICAL DEMANDS:

  • Ability to perform extensive walking, standing, and sitting, with some bending, kneeling, and reaching.
  • Capability to lift up to 35 pounds and assist with wheelchair mobility.

MENTAL AND EMOTIONAL REQUIREMENTS:

  • Ability to follow oral and written instructions and prepare detailed reports.
  • Capability to work independently or as part of a team under varying levels of supervision.
